    Plaquenil and procedures

Plaquenil is less toxic than chloroquine; however, long-term use of either drug can result in macular toxicity, leading to devastating irreversible vision loss. The mechanism of this toxicity is not clearly understood, though it is believed that the drug molecule binds to melanin in the retinal pigment epithelium (RPE).

Plaquenil ® hydroxychloroquine sulfate can potentially interact with a few other medications. Some of these medicines that may lead to Plaquenil drug interactions include Certain beta blockers, such as Bisoprolol Zebeta ® Metoprolol Lopressor ®, Toprol XL ® Propranolol Inderal ® Sotalol Betapace ® Timolol Blocadren ® Plaquenil does not need to be stopped for surgery so it can be restarted after the procedure. Although in studies Celebrex does not cause increased bleeding, unilke other older NSAIDs, some surgeons still advise to stop it to be safe. In general, Celebrex can be restarted the day after the procedure once the risk of bleeding is over. Plaquenil is the brand name for the prescription drug hydroxychloroquine. It's used to treat and prevent malaria infection, and to reduce symptoms and progression of autoimmune diseases such as lupus, rheumatoid arthritis, and others.

Binding of the drug to melanin in the RPE contributes to, or prolongs, its toxic effects. This leads to disruption and damage to the photoreceptors and outer nuclear and plexiform layer, sparing the foveal center and resulting in the “bull’s eye” appearance in the late stage of the disease.
