    Plaquenil peripheral neuropathy

These neuropathies can have different causes and may require different diagnostic techniques & therapeutic strategies. Unlike other autoimmune disorders, in which the neuropathies predominantly cause weakness, the neuropathies in Sjögren’s primarily affect sensation and can cause severe pain.

Plaquenil and small fiber neuropathy I used Plaquenil years ago for extreme muscle and joint issues and it seemed to work fine. All my issues were resolved over a years time on low doses. Jan 25, 2010 You might want to click on the Drugs A to Z tab at the top of this page. You can find out lots of information on peripheral neuropathy. I read some of it and I had two bouts of pancreatitis and one of the comments was that Zalcitabine can cause severe peripheral neuropathy and can also cause pancreatitis. Including our patient, there are 10 reported cases of HCQ neuromyotoxicity. Muscle biopsy consistently reveals curvilinear bodies and muscle fiber atrophy with vacuolar changes. Most cases manifest as insidious onset proximal myopathy that may be associated with peripheral neuropathy and cardiac myotoxicity.

Just as most causes of neuropathies and neuropathic pain in Sjögren’s do not come on suddenly, reduction of neuropathic pain can take a while. Initial and predominant neuropathies in Sjögren’s can occur anywhere in the feet, thighs, hands, arms, torso and/or face. Many different symptomatic therapies for neuropathic pain are available. Recognition of unique patterns & causes of neuropathies in Sjögren’s is important in arriving at appropriate therapies. Recognize that neuropathic pain is a chronic disease.

My understanding is that the rheumatologist thinks that plaquenil is synergistic with mycophenolate and the 2 have a better chance of halting the progression of the neuropathy, and the other current and possible impacts of Sjogren’s better than mycophenolate alone. Jan 22, 2013 Acquired peripheral neuropathies are caused by systemic disease, trauma from external agents, or infections or autoimmune disorders affecting nerve tissue. Inherited forms of peripheral neuropathy are caused by inborn mistakes in the genetic code or by new genetic mutations. Small-fiber neuropathy is one of the most common types of peripheral neuropathy affecting patients with systemic lupus erythematosus, but it isn’t even mentioned in the American College of Rheumatology neuropsychiatric case definitions of manifestations of the disorder, according to a retrospective analysis of cohort of 2,097 patients with SLE.
